About the Role

We're seeking a dedicated and compassionate Registered Nurse to join our skilled nursing facility in Pawtucket, Rhode Island. As a key member of our healthcare team, you'll be responsible for delivering high-quality patient care, collaborating with colleagues, and contributing to a positive and supportive work environment.

Responsibilities

• Provide direct patient care, including administering medications, treating wounds, and monitoring vital signs
• Develop and implement individualized care plans to meet the unique needs of each resident
• Collaborate with other healthcare professionals, including physicians and therapists, to ensure comprehensive care
• Monitor and report changes in resident condition, including adverse reactions to medications or treatments
• Participate in quality improvement initiatives to drive positive outcomes and reduce readmissions
• Document patient information accurately and maintain confidentiality

Requirements

• Current licensure as a Registered Nurse in the state of Rhode Island
• 0 years of experience in a long-term care or rehabilitation setting
• Strong communication and interpersonal skills
