Seeking Physical Therapists (full-time & part-time) ~ Blacksburg, VA

 
American HealthCare, LLC, the management company for Heritage Home Health , is seeking Physical Therapists to join our dynamic team in our start-up home health agency, located in Blacksburg, Virginia . Candidates must have a minimum of 1 year of home health experience, be well-organized, patient-focused and possess strong communication and clinical skills.
Key responsibilities include, but are not limited to :

• Developing and implementing appropriate, individualized care plans for homebound patients.
• Evaluating and providing interventions to improve balance, gait, strength, mobility and functional independence.
• Participating in interdisciplinary team meetings.
• Directing and supervising Physical Therapy Assistants and Home Health Aides.

We are seeking individuals with the following principal qualifications :

• Licensed to practice Physical Therapy in the Commonwealth of Virginia
• Minimum 1 year of home health experience
• Valid driver's license and clean driving record
• Strong clinical, interpersonal and communication skills
• Timely and accurate reporting & documentation skills
• Successful completion of a background check
• Ability to work effectively with the clinical team
