Physical Therapist (PT) - Per Diem/PRN

Responsibilities Evaluates assigned patients and establishes appropriate treatment goals and programs. Provides ongoing evaluation of patient status and goals, modifying treatment program for timely progression. Develops comprehensive goals for patient treatment to maximize outcomes and return to community activities. Coordinates treatment program with other clinical/therapeutic services and available community resources; attends appropriate clinical meetings to facilitate coordination. Provides patient education, including development of progressive home programs and instructions for patient, family, and caregivers. Prepares for patient discharge, including ordering necessary equipment and recommendations for therapeutic services to ensure continuum of care. Evaluates patient in actual or simulated home, school, work, or community environments to facilitate return to community activities, making recommendations for adaptations or training. Identifies and manages various levels of care needed to ensure care plan delivery; delegates and supervises PTAs and support personnel to maximize outcomes. Minimum Requirements Current state licensure or eligibility for state licensure as a Physical Therapist. Certified BLS. Per Diem Requirement Two (8_hour) shifts per month.
