Responsibilities
- Conduct comprehensive OT evaluations and develop individualized treatment plans for adult residents in an acute care context within a skilled nursing setting.
- Implement evidence-based OT interventions to improve ADLs, mobility, upper-extremity function, cognition, safety awareness, and functional transfers.
- Monitor progress, reassess outcomes, and document patient status and treatment progression in accordance with regulatory and organizational guidelines.
- Collaborate with an interdisciplinary team to coordinate discharge planning, transitions of care, and goal alignment across disciplines.
- Educate patients and families on home programs, safety strategies, adaptive equipment, and activity modification; provide training to staff as needed.
- Ensure safety, infection control, and adherence to relevant policies while delivering high-quality, patient-centered therapy.

Requirements
- Current occupational therapy license or eligibility to practice in the United States; ability to travel and work in a travel assignment.
- 1+ year of clinical experience in acute care or post-acute rehab settings preferred.
- Strong documentation skills and ability to communicate effectively with patients, families, and the care team.
- Ability to work in a fast-paced, interdisciplinary environment and manage a varied caseload.
- Availability to start on 07/20/2026 and commit to approximately 13 weeks.
- Must be able to work 40 hours per week in a travel assignment.
