Key Responsibilities Conduct comprehensive psychosocial assessments through direct interviews, patient observations, and detailed reviews of medical records to evaluate emotional, social, and environmental factors influencing patient recovery. Deliver individualized counseling and emotional support to patients and their families, effectively addressing adjustment issues and fostering a positive psychological outlook. Work integratively with the rehabilitation team, which includes physical therapists, occupational therapists, and speech-language pathologists, to develop and implement tailored treatment plans addressing each patient's unique psychosocial needs. Assist patients in navigating community resources, such as vocational rehabilitation programs, support groups, and housing assistance, to empower them in their rehabilitation journey. Actively participate in discharge planning to facilitate smooth transitions for patients returning home or moving to different care settings, ensuring continuity of support and care. Provide educational resources and training to patients and family members about available community resources and long-term post-rehabilitation services. Patient Population / Client Focus The Clinical Social Worker will engage with a diverse population of patients dealing with conditions such as stroke, spinal cord injuries, orthopedic surgeries, and neurological disorders. These patients often require sensitive emotional support and advocacy as they navigate significant life changes. The ideal candidate will be dedicated to facilitating positive outcomes, promoting independence, and enhancing the overall well-being of patients through effective therapeutic relationships. Qualifications A Master’s degree in Social Work from an accredited program is required. Possession of an active state licensure as a Licensed Clinical Social Worker (LCSW) is essential to practice in this role. Strong communication skills paired with excellent interpersonal capabilities to build rapport and trust with patients and their families. A deep understanding of psychosocial adjustment issues, grief, and coping strategies is crucial for effective practice. Preferred Experience Experience working within a rehabilitation or medical setting is strongly preferred, offering insights into the dynamics of recovery processes. Familiarity with evidence-based treatment methodologies related to psychosocial support and emotional health will be an asset. Compensation and Benefits This position offers an attractive salary range of $75,000 to $100,000 per year, commensurate with experience.
