Role Overview: The Home Health Occupational Therapist Assistant provides personalized therapy services to help patients regain strength, mobility, and independence in their homes. They work under the supervision of a qualified Occupational Therapist, implementing treatment plans, training patients and families in daily skills, documenting progress, and participating in care coordination and discharge planning. Key Responsibilities Deliver therapy and educational activities based on the patient's Plan of Treatment Train patients and families in activities such as dressing, feeding, grooming, and hygiene Maintain accurate documentation of patient progress and clinical notes Collaborate with healthcare team members to meet patient needs and plan discharges Qualifications & Skills Current and unrestricted Occupational Therapy Assistant license in North Carolina Valid driver’s license, auto insurance, and reliable transportation Current CPR certification Preferred: Home health experience Additional Requirements: Must adhere to driver safety and TB screening policies.
