We believe in maintaining a culture of wellness that prioritizes the overall health, happiness, and satisfaction of our team members.Key ResponsibilitiesConduct thorough evaluations of patients recovering from orthopedic injuries, surgeries, or chronic conditions in their home setting.Develop and implement comprehensive treatment plans that utilize a variety of therapeutic techniques, such as manual therapy, therapeutic exercises, and technological modalities.Motivate, educate, and guide patients through their rehabilitation processes, ensuring clarity on goals and expectations.Continuously monitor patient progress and adjust treatment strategies as necessary, collaborating with other healthcare professionals to optimize outcomes.Track patient treatment documentation accurately and in compliance with regulatory standards, maintaining thorough records on treatment progress and patient responses.Advise on the selection and usage of adaptive equipment to enhance patient independence in self-care and mobility.Engage in professional development by attending workshops, reviewing literature, and contributing to community health initiatives.Patient Population / Client FocusOur primary client focus encompasses patients of all ages experiencing orthopedic conditions, ranging from post-operative rehabilitation to chronic pain management. As an orthopedic home care physical therapist, you will work closely with each patient to develop personalized care strategies that respect their unique circumstances, preferences, and goals, ensuring the highest degree of patient satisfaction and engagement.QualificationsBachelor's degree in Physical Therapy; additional education such as a Doctorate in Physical Therapy (DPT) is preferred.Must possess a valid New York State license for Physical Therapy.Exceptional written and verbal communication skills to interact effectively with patients, families, and interdisciplinary teams.Proficient in technology usage including electronic medical records (EMR) systems and standard office software.Ability to demonstrate flexibility and adaptability in a dynamic home care setting.Preferred ExperienceA minimum of two years of experience in physical therapy practice, particularly in orthopedic rehabilitation or home health care.Experience working with multidisciplinary health teams and an understanding of comprehensive care models.Previous exposure to mentorship or supervision roles is a plus, especially for recent graduates looking to transition into the field.Compensation and BenefitsThe salary for this position ranges from $90,000 to $100,000 annually, commensurate with experience and qualifications.
