This position relies on relationship-building skills to ensure we maintain close relationships with referral sources and build strong relationships with new referral sources.If you are a strong leader who enjoys connecting with people, thrives in a dynamic environment, and wants to make a measurable impact on patient care and agency growth, this role is for you. **Key Responsibilities** * Manage incoming referrals from hospitals, skilled nursing facilities (SNFs), case managers, and other partners.* Generate new referral sources.* Perform patient intakes and execute service agreement contracts as needed.* Coordinate care initiation promptly to reduce delays and improve patient experience.* Maintain accurate referral and intake trackers.* Build and maintain strong relationships with referral sources through regular communication and check-ins.* Generate new referrals from established referral sources.* Identify opportunities for growth and assist with marketing initiatives as needed.* Serve as a liaison between referral sources, patients, and internal care teams. **Qualifications*** Strong organizational and time-management skills.* Excellent communication and interpersonal skills.* Ability to work independently and manage multiple priorities.* Previous experience in home health, personal care, or case management preferred.* Marketing or referral development experience is a plus (but not required). **Why Join Stonebridge?*** Full-time Community Liaison role with opportunity to grow into a Director of Marketing role as Stonebridge continues to grow.* Supportive team environment focused on collaboration and professional development. **Compensation*** Hourly pay, with mileage reimbursement for intakes and referral check-ins (details discussed during interview). **Ready to make a difference?
