Patient Engagement Specialist

Position Overview Under general supervision, the Patient Engagement Specialist I provides patient engagement services to recruit patients into the program’s system of care. The specialist supports the planning, development, implementation, and evaluation of services in accordance with contractual and departmental requirements and guidelines. Responsibilities Develops, identifies, and provides referrals within and outside of FHCSD, and monitors and tracks these referrals. Communicates effectively with and recruits new patients into the clinic’s or program’s system of care, interacting well with community members and patients from diverse social, cultural, and economic backgrounds. Leverages community resources to raise awareness of FHCSD, establishes and maintains good working relationships with community partners, and uses those resources to connect new and existing patients with FHCSD. Participates in and supports the planning, development, execution, and evaluation of patient engagement plans. Investigates leads for patient recruitment and engagement opportunities. Produces accurate and timely written materials such as plans, flyers, reports, and other documents. Uses computer resources as required to meet organization’s demands. Performs other duties as assigned. Requirements Bachelor’s degree strongly preferred; 4 years of recent, relevant work experience may substitute for the degree. High school diploma or GED equivalent required. At least 1 year of experience in sales, patient engagement, community education, community health, or outreach programs. Specific experience in the program area may be required depending on assignment. Bilingual (read, speak, and write) in English and Spanish required. Meet organizational driving requirements. Benefits and Compensation Job type: Regular Full Time. Competitive salary range: $25.00 – $30.93 per hour.
