Brookestone Home Health Care is seeking a compassionate and dedicated Physical Therapist Assistant (PTA) to join our team serving Elkhorn and the surrounding communities. As a PTA, you will work directly with patients in their homes, helping them improve mobility, regain independence, and achieve their rehabilitation goals under the direction of a licensed Physical Therapist.

Position Summary

The Physical Therapist Assistant provides physical therapy services as prescribed by a physician and directed by a licensed Physical Therapist. The PTA implements treatment plans, monitors patient progress, and collaborates with the interdisciplinary care team to support positive patient outcomes.

Essential Duties and Responsibilities

• Administer physical therapy treatments according to the established plan of care.
• Observe and report patient progress, responses, and concerns to the supervising Physical Therapist.
• Educate patients and caregivers on exercise programs, mobility techniques, and home safety.
• Maintain accurate, timely, and compliant clinical documentation.
• Participate in care coordination with physicians, nurses, therapists, and other healthcare professionals.
• Promote patient independence, safety, and quality of life.
• Travel to patient homes within the assigned service area.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current Nebraska PTA license in good standing.
• Current CPR certification.
• Valid driver's license, dependable transportation, and proof of automobile insurance.
• Strong communication and organizational skills.
• Ability to work independently while maintaining close collaboration with the supervising therapist.
• Home health experience preferred, but not required.
