RN - Complex Discharge Planner - WCMC - AS Utilization Management - Full Time - 8 Hour - Days

Job Description The Complex Discharge Planner plays a vital role in managing patient flow from Concord Medical Center (CMC) and Walnut Creek Medical Center (WCMC) to post-acute care settings, including Skilled Nursing Facilities (SNFs), Residential Care Facilities for the Elderly (RCFEs), board‑and‑care homes, and other lower levels of care. This position supports system-wide efforts to reduce hospital length of stay by ensuring timely transitions, leasing post-acute beds when needed, and helping patients access benefits and resources that support safe discharge and long-term success. The RN also plays a key role in ensuring that patients receive the right care, at the right time and place. The Complex Discharge Planner has a relentless focus on excess day reduction and care transitions when patients do not require general acute care. This role is utilized for community members who require a higher level of case management than typical post-acute services, helping to manage demand across the care continuum. Responsibilities and Duties Manage patient flow from acute care hospitals to post-acute care settings. Ensure timely transitions and coordinate leasing of post-acute beds. Assist patients in accessing benefits and resources for safe discharge and long-term success. Focus on excess day reduction and care transitions for non-acute patients. Provide higher-level case management for community members across the care continuum. Work Details Shift: 08:00 – 16:30 (40 hours per week) Pay Range: $96.12 – $129.76 per hour (based on experience, licensure, and local market adjustments). Location: John Muir Health – Concord Medical Center (CMC) and Walnut Creek Medical Center (WCMC), Contra Costa, Alameda and southern Solano Counties. Qualifications Education : Bachelor of Science in Nursing (BSN) – required Experience : At least 3 years of clinical acute care experience, with 1+ year in case management, discharge planning, or post-acute care coordination (Skilled Nursing, Residential Care, or Payor); preferred experience in leased bed programs, patient throughput management, letters of agreement, or transitional care coordination; preferred experience working with diverse and underserved populations. Certifications : RN Registered Nursing – California Board of Nursing (required); BLS Basic Life Support – American Heart Association (required); Case Management Certification (CCM, ACM, or similar) – preferred. Knowledge, Skills & Abilities : In‑depth understanding of post-acute care services and hospital discharge processes; familiarity with Medi‑Cal and other benefit programs; excellent communication, critical thinking, financial analysis, time management, and continuous improvement skills; ability to build strong relationships with physicians, interdisciplinary teams, patients, families, and community partners; proficiency in Epic Electronic Health Record.
