Job Title: Home Health Physical Therapist

Experience: Minimum of 1 year of skilled home health experience
License: Pending license accepted

Must-Have: Oasis experience required


Description:
Home Health Physical Therapist needed to work in East Portland and surrounding areas.
The start date is as soon as possible.
This position requires 8-hour day shifts.
This facility does not accommodate 4x10 or 3x12 schedules.
The therapist will have a 1:1 patient ratio.
The therapist may visit up to 6 patients per day.
A productivity standard of 30 points is expected.
First-timers are welcome to apply.
Weekend work is not required.
No on-call duties are expected.
Travelers are expected to have their own transportation.
The facility effectively groups clients to minimize drive time.
Occasional travels may extend to a maximum radius of approximately 60 miles.
Business casual attire is preferred; scrubs are acceptable but not required.
Charting will be done using EPIC Remote Client and Rover applications.
Patient age groups include adults and geriatrics.
Daily patient census averages 5.5 - 6 points per day.
The unit treats various diagnoses including dementia, Parkinson's, ALS, MS, and post-operative ortho cases.
Safety and injury prevention practices are essential.
All travelers may be asked to float between facilities within the state.
