Family Medicine NP/PA - New Hartford

The Nurse Practitioner (NP)/Physician Assistant (PA) is responsible for providing competent medical care in accordance with the professional scope of practice and current regulatory statutes. As a NP/PA you will be expected to perform hospital consults and rounds, take patient's history, perform physical examinations, perform procedures (within scope of practice) under the supervision of a licensed physician, treatment planning within the scope of practice and interpret physical, psychological and preventive health data to determine patient's needs and problems. Able to prescribe medications as allowed by scope of practice and pharmacy formulary.

Core Job Responsibilities:

• Performs physical exams on all patients, neonate through the geriatric population.
• Obtains and documents patient's medical history.
• Identifies health risk factors.
• Diagnoses and treats common acute and chronic illnesses or injuries.
• Orders and interprets diagnostic tests, i.e., laboratory work, x-rays, EKG.
• Prescribes medications per state laws.
• Prescribes non-pharmacological therapies based on the patient's diagnoses, health history and age.
• Encourages positive health behaviors for health maintenance and the prevention of disease through education and counseling.
• Always applies standardized care guidelines in clinical practice.
• Assesses and documents patient and/or family psychosocial issues.
• Works with the patient and family to formulate a patient treatment and education plan.
• Provides counseling and referrals to other healthcare professionals and community resources when appropriate.
• Collaborates with physicians and other members of the health team to plan optimal care for the patient.
• Responds appropriately in emergent situations
• Provides follow-up care, determines effectiveness of the treatment plan, reassesses and changes the plan as needed.
• Document all findings, interventions and results.
• Maintains patient confidentiality at all times.
• Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
• Provides age appropriate, culturally sensitive care to the assigned patient population.
• Stays current with medical advancements, new technology, new drugs through continuing education classes, society meetings, professional journals etc.
• Follows the five medication rights and reduces the potential for medication errors.
• Treats patients and families with respect and dignity.
• Attends all departmental, organizational and medical staff committee meetings as appropriate.
• Perform other duties as required.

Education/Experience Requirements:

• REQUIRED: Master's degree from an accredited Nurse Practitioner's Program OR certification from an accredited NP program OR completion of an accredited Physician Assistant Program required. Minimum 1 year of experience in a clinical setting.
• Ability to meet and maintain credentialing requirements and hospital privileges.
• Knowledge and competency in a wide range of clinical areas.
• Familiar with EHR/EMR software.

Licensure/Certification Requirements:

• REQUIRED: Current NYS Nurse Practitioner licenses OR Current NYS license as a Physician Assistant with prescription privileges. Current BCLS, ACLS certifications.
• PREFERRED: Active ANCC certification in relevant specialty
