Requirements:

• Proficient in METs testing evaluating and diagnosing respiratory and cardiovascular - REQUIRED
• Own a laptop (no Mac/Apple products) - REQUIRED
• AANP - REQUIRED
• Active and unrestricted full license - REQUIRED
• Willing to complete DMA training to become DMA certified - REQUIRED
• CV needs current home address listed for presentation - REQUIRED
• License number and Certification Number must be provided - REQUIRED
• Willing to perform Gen/Med (yes) - REQUIRED
• DMA certified (yes/no) - REQUIRED
• Home address must be provided - REQUIRED
• VES location provider is applying to - REQUIRED
• Open to travel to other locations? - REQUIRED
• Provider must have own office address if applicable - REQUIRED
• Self-Query NPDB should be pulled within the month - REQUIRED
• Active military duty or VA employment status - REQUIRED
