Healthcare Provider - Nurse Practitioner / Physician Assistant

Join a dedicated primary care team as a Nurse Practitioner, delivering high-quality, patient-centered care to seniors in a value-based setting. Responsibilities include evaluating and treating patients, coordinating services such as specialty referrals and hospitalizations, participating in team huddles, and ensuring timely documentation. The role involves following clinical policies, supporting quality improvement, and engaging in community relationships. The NP will work closely with the care team to enhance patient outcomes, spend meaningful time with patients, and participate in on-call programs.
