Hospice Case Manager RN: Lead End-of-Life Care
Advocate Health Care
Advocate Health Care is seeking a registered nurse to coordinate hospice patient and family care, provide end-of-life comfort, and maximize quality of life across hospice settings. You will lead the interdisciplinary care group to create and update the Plan of Care, document in the electronic health record, and supervise Home Health Aide and licensed practical nurse visits.
