Licensed Practical Nurse, LPN - Home Health
PRN/Per Diem | Hillsborough County, FL
Trilogy Home Healthcare , a Humana company, is hiring a dedicated and compassionate Licensed Practical Nurse, LPN to join our award-winning team. - Provide high-quality, compassionate care to home health patients according to patients plan of care
- Use assessment tools to manage medical needs and coordinate with other disciplines
- Serve as a patient advocate, communicating with physicians, clinical managers, and RNs
- Make a meaningful impact on patients' lives every day

What You Bring:
- Graduate of an accredited school of nursing
- Active LPN license in Florida
- Valid Florida drivers license, active vehicle insurance, and clear MVR
- Minimum 1 year nursing experience required; Home Health experience is a plus
- IV certification preferred
- Strong initiative and independent judgement
