Conduct psychosocial assessments and identify barriers to discharge; coordinate follow‑up as needed. Develop and implement transition plans; facilitate post‑discharge planning with outside agencies, nursing staff and relatives. Offer direction for routine non‑medical problems affecting patients and families and assist with resource navigation. Document interactions, maintain accurate records, and communicate progress with the multidisciplinary team. Requirements Active California social work license or eligibility to obtain licensure. Master of Social Work (MSW) degree from an accredited program. Experience or strong familiarity with medical/inpatient social work and discharge planning. Demonstrated counseling, communication and collaboration skills within a multidisciplinary team. Ability to work on‑site in an inpatient care setting and handle sensitive family interactions.
