Responsibilities
- Provide counseling, support and assistance to patients and families to address needs incidental to hospitalization.
- Conduct psychosocial assessments and identify barriers to discharge; coordinate follow‑up as needed.
- Develop and implement transition plans; facilitate post‑discharge planning with outside agencies, nursing staff and relatives.
- Offer direction for routine non‑medical problems affecting patients and families and assist with resource navigation.
- Document interactions, maintain accurate records, and communicate progress with the multidisciplinary team.

Requirements
- Active California social work license or eligibility to obtain licensure.
- Master of Social Work (MSW) degree from an accredited program.
- Experience or strong familiarity with medical/inpatient social work and discharge planning.
- Demonstrated counseling, communication and collaboration skills within a multidisciplinary team.
- Ability to work on‑site in an inpatient care setting and handle sensitive family interactions.
