Key Responsibilities

• Conduct thorough patient assessments, including detailed physical exams and health evaluations.
• Diagnose and manage both acute and chronic health conditions, collaborating closely with the care team to devise effective treatment strategies.
• Create individualized treatment plans tailored to meet patient needs, which may involve prescribing medications or recommending therapies.
• Continuously monitor patient progress, adjusting care plans as necessary to optimize health outcomes.
• Provide education to patients and their families regarding medication management, treatment options, and preventative care strategies.
• Maintain meticulous and up-to-date patient records in compliance with HIPAA regulations to ensure confidentiality and proper documentation.
• Work alongside multidisciplinary teams to create and sustain a seamless continuum of high-quality care for all patients.

Patient Population / Client Focus

Your primary focus will be on a diverse range of patients, including children, adults, and seniors with various medical needs. You will address both routine preventive care and the management of complex health conditions, ensuring that every patient receives attentive and personalized care. Your ability to build rapport with patients and effectively communicate treatment plans will be vital to their health management and overall experience.

Qualifications

• Must possess an active Nurse Practitioner (NP) license in the state of Florida.
• A Master of Science in Nursing (MSN) with a specialization in Nurse Practitioner studies is required.
• Board certification as either a Family Nurse Practitioner (FNP), Adult Nurse Practitioner (ANP), or Psychiatric-Mental Health Nurse Practitioner (PMHNP) is essential.
• A valid DEA license and NPI number.
• Florida Board of Pharmacy license is mandatory.

Preferred Experience

• At least two years of experience working in outpatient, hospital, or primary care settings is highly preferred.
• Experience in managing chronic diseases and familiarity with electronic health record (EHR) systems to document patient interactions effectively.
• Proficiency in collaborating with healthcare teams to enhance patient outcomes and care coordination.

Compensation and Benefits
