Pay Rate: $2,300.00 - $2,500.00 gross per week Number of Positions: 1 Specialty : Rehab Start Date: 7/20/2026 Location: Aspirus at Home - Wisconsin Rapids Travel : All inclusive Assignment Length: 13 weeks EMR : Epic Schedule : 40 hours per week Monday - Friday 8a - 4:30p 30 minute lunch is required 38 hours - flex to 40 guaranteed Holiday Rotation: none Will rotate 1 weekend every 6-8 weekends Meets productivity expectations of an average of 4.5 visits per day Additional Details about the position Charting/Equipment: Laptop for point of service documentation, NetSmart EMR, and cell phone for GPS and communication. Unit Specific Skillset/Unit Overview: Evaluates, treats and plans patient care in accordance with Physical Therapy Standards of practice. Population served includes adult and geriatric patients. Documents to meet regulatory and reimbursement requirements Hard worker, Resilient and Manages Change well, Social candidates. Scrub Colors: any or Business casual EXPERIENCE REQUIREMENTS Minimum Experience: Home Health experience, training working with the adult/geriatric population with a variety of orthopedic, neurological and functional mobility diagnosis. Knowledge of Physical Therapy standards and practices acquired through a minimum of a Masters degree in Physical Therapy.
