Job Description

We are seeking a highly experienced Acute Inpatient Case Manager (RN) to join our dynamic healthcare team. The ideal candidate will possess a strong background in acute inpatient case management and discharge planning, demonstrating the ability to quickly adapt and perform efficiently in a fast-paced environment.

Responsibilities

• Manage a case load of 15-20 patients across all inpatient units.
• Coordinate with the social work team to facilitate patient discharges.
• Engage in acute discharge planning and complex placements.
• Utilize strong computer skills and Epic EMR for documentation and case management.
• Ensure compliance with criteria for financial assistance for uninsured or underinsured patients.

Required Experience / Certifications / Licensure

• Minimum of 3 years of acute inpatient case management experience, including at least 1 year in a large hospital (500+ beds, LEVEL 1ACADEMIC Medical Center).
• Strong computer skills with experience in Epic EMR.
• Nursing degree (RN preferred; BSN or ADN considered).
• BLS certification through AHA required; CCM preferred but not mandatory.
• Recent acute inpatient case management experience within the last two years.
