Position Summary
The organization is seeking a Family Nurse Practitioner to join its expanding outpatient primary care team in Southbury, Connecticut.
This full-time, permanent role is outpatient-focused and offers a supportive clinical structure with experienced physicians, advanced practice providers, and ancillary staff. Providers benefit from a patient-centered culture guided by academic best practices.
New graduates and experienced FNPs are encouraged to apply!
Essential Responsibilities
- Provide comprehensive primary care services to patients across the lifespan
- Diagnose and manage acute and chronic medical conditions
- Conduct preventive care visits, annual physicals, and wellness screenings
- Prescribe medications and develop evidence-based treatment plans
- Collaborate with physicians and multidisciplinary care teams
- Maintain accurate and timely EMR documentation
- Educate patients on health maintenance and disease prevention
Qualifications
- Education: Master's or Doctoral degree from an accredited FNP program
- Certification: National certification as a Family Nurse Practitioner
- Licensure: Eligible for Connecticut APRN licensure
- Experience: Prior primary care experience preferred; new graduates welcome
