Physical Therapist Case Manager, Level 2: Home Health

Job Title and Location Full Time Physical Therapist Case Manager, Level 2 Anticipated Territory: Windsor, CA Area Responsibilities Perform evaluations in patients’ homes and develop individual care plans. Work with Physical Therapy Assistants (PTAs) to follow care plans. Help patients improve mobility and regain independence. Communicate and coordinate with physicians to ensure quality of care. Evaluate and report patients’ progress to medical directors. Supervise and mentor 2 PTAs, ensuring they meet performance standards. Qualifications Current unrestricted CA Physical Therapist License. Current BLS/CPR Certification. Valid CA Driver’s License. Knowledge of local community resources required. Understanding of illness/injury impact on development and behavior, including cultural influences on healthcare. Knowledge of state and federal home‑care regulatory guidelines preferred. Experience in home‑health care highly preferred. Experience with high‑acuity patients a plus. Experience working with an interdisciplinary team highly desired. Case management fundamentals desired. Self‑directed, empathetic, critical thinker with strong interpersonal and leadership skills and organized. Benefits Concierge service: assistance with documentation and administrative tasks. Wage range: $125,000.00 – $160,000.00 plus $20,000 annual bonus, paid quarterly.
