We are seeking a Tuesday Night 12 HR LVN Per Diem Night Shift Available to join our team! You will be responsible for delivering high quality care to assigned patients.
Responsibilities:- Care for long term disabled population
- Provide care to subacute population including trach and ventilator population
- Medication pass and treatment nurse to 16 patients with feeding tube
- Supervise and collaborate with CNAs on your shift
- Document in EHR using PCC software
Qualifications:
- Subacute facility experience prefered
- Familiarity with trach/vent patients prefered
- Excellent time managment skills required
- Compassionate and caring demeanor
- Excellent written and verbal communication skills
- Ability to work independently and as a team required
We are a 16-bed congregate living health facility. We offer 12 hour shifts.
