Position Summary

The Physical Therapist (PT) evaluates, develops, and implements individualized therapy programs designed to restore mobility, reduce pain, improve function, and maximize each resident's independence. Working collaboratively with residents, families, physicians, nursing staff, and the interdisciplinary care team, the Physical Therapist provides clinical leadership while delivering evidence-based rehabilitation services that promote exceptional outcomes and quality of life.

Key Responsibilities
  • Perform comprehensive physical therapy evaluations and develop individualized treatment plans based on resident goals and physician orders.
  • Deliver evidence-based therapeutic interventions to improve strength, balance, mobility, endurance, pain management, and functional independence.
  • Reassess resident progress and modify treatment plans to maximize clinical outcomes.
  • Supervise and provide clinical direction to Physical Therapist Assistants (PTAs) and Rehabilitation Technicians.
  • Educate residents and families on treatment plans, therapeutic exercises, adaptive techniques, fall prevention, and strategies to maintain independence.
  • Collaborate with physicians, nursing staff, therapy professionals, and the interdisciplinary care team to coordinate comprehensive resident care.
  • Maintain accurate, timely, and compliant clinical documentation in accordance with professional standards and regulatory requirements.
  • Participate in care conferences, discharge planning, quality improvement initiatives, infection prevention programs, and interdisciplinary meetings.
  • Assist with staff education, mentorship, orientation, and competency development.
  • Maintain therapy equipment and treatment areas to ensure a safe, clean, and organized environment.

Requirements

Required Qualifications
  • Doctorate or Master's degree in Physical Therapy from an accredited program.
  • Current Physical Therapist (PT) license in the State of Kentucky.
  • Current CPR certification (or ability to obtain within required timeframe).
  • Strong knowledge of physical therapy evaluation, treatment planning, and rehabilitation techniques.
  • Excellent communication, interpersonal, leadership, and organizational skills.
  • Ability to supervise and collaborate effectively with Physical Therapist Assistants and Rehabilitation Technicians.
  • Commitment to providing compassionate, resident-centered care.
  • Ability to meet the physical demands of the position, including lifting up to 50 pounds, prolonged standing, walking, bending, transferring residents, and assisting with therapeutic activities.
Preferred Experience
  • Previous experience in skilled nursing, long-term care, inpatient rehabilitation, or hospital settings.
  • Experience treating geriatric and post-acute rehabilitation populations.
  • Knowledge of Medicare Part A, Part B, PDPM, and rehabilitation documentation requirements.
  • Experience supervising therapy staff and collaborating with interdisciplinary healthcare teams.
  • Strong clinical reasoning, documentation, and time management skills.
  • Passion for helping residents regain independence and improve their quality of life.
