You will educate providers and families, build referral relationships, coordinate admissions, and assess needs and barriers to care. This role combines relationship-building, clinical coordination, and sales strategy to improve patient outcomes. A few position highlights include:* Educate health care professionals, patients, and families about home care and senior living services.* Develop and maintain relationships with key referral sources, such as hospitals, nursing homes and physician offices to generate leads.* Coordinates patient admissions with the team, including obtaining all required documentation to complete referral to admission process.* Track leads, document activity in CRM, meet admissions and account goals, attend community events, and respond to customer concerns.**THE MUST HAVES:*** At least two (2) years of experience in a healthcare related business development or community outreach role.
