What You’ll Do Travel to homes, assisted living, and care communities to complete hospice admissions. Perform eligibility assessments and initiate hospice services . Lead conversations around goals of care, expectations, and next steps . Build trust quickly with patients, families, and referral partners. Coordinate closely with intake, physicians, and the interdisciplinary team. Ensure a smooth, compassionate transition into hospice care . Complete accurate, timely EMR documentation . What We’re Looking For Experience in hospice, oncology, palliative care, or hospital-based end-of-life care . Strong ability to lead conversations and build rapport quickly . Confident clinical assessment and decision-making skills. Comfortable working independently in a mobile, field-based role . Reliable transportation and ability to travel throughout the metro.
