Description

• Perform evaluations for patients in both acute and outpatient environments and craft personalized treatment plans
• Implement physical therapy techniques with a focus on enhancing mobility, strength, balance, and overall function
• Work alongside nurses, physicians, case managers, and other rehabilitation professionals to optimize patient outcomes
• Guide patients and their families on therapeutic objectives, discharge processes, and at-home exercises
• Document patient evaluations, treatment sessions, and progress following facility and regulatory protocols
• Engage in interdisciplinary conferences and care team discussions to ensure seamless care

Education

• Doctorate in Physical Therapy (DPT) or equivalent education from an accredited institution

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification preferred
• Additional specialized certifications (such as neurology or geriatrics) welcome but not mandatory

Skills

• Prior experience in a related role is highly desirable
