Details Skilled Nursing Facility (SNF) setting, focused on inpatient, long-term care, and short-term rehabilitation Patient population includes geriatric residents requiring mobility restoration, functional improvement, and independence optimization Day shift schedule: 9:00 AM – 5:00 PM 13-week contract Requirements Graduate of an accredited Physical Therapy program required Current, active state PT license required; license required if applicable CPR/BLS certification preferred Experience in a SNF or geriatric rehab setting preferred Strong communication, documentation, and interpersonal skills required Ability to perform physical tasks including lifting, standing, walking, and assisting patients with transfers and mobility DOB and SSN may be required for consideration Additional Information Responsibilities include evaluating residents' physical conditions, developing individualized treatment plans, and delivering evidence-based rehabilitative interventions Collaborate with an interdisciplinary team including physicians, nursing staff, occupational and speech therapists, and family members Monitor and document patient progress in accordance with state, federal, and Medicare/Medicaid guidelines Educate patients and caregivers on safe mobility techniques, home exercise programs, and adaptive equipment use Supervise physical therapist assistants (PTAs) and aides as needed Participate in care planning meetings and interdisciplinary team conferences No COVID-19 vaccine requirement for this position American Traveler Job ID P-690689.
