- Coordinate and oversee patient care plans to ensure continuity and quality of care.
- Perform patient assessments, including psychosocial, medical, and functional evaluations.
- Collaborate with interdisciplinary teams to develop individualized care plans and monitor patient progress.
- Act as a liaison between patients, families, and healthcare providers, facilitating effective communication and resource coordination.
- Advocate for patient needs while ensuring compliance with healthcare regulations and standards.
- Maintain accurate documentation of patient care activities in accordance with facility policies and regulatory requirements.
- Educate patients and families on care plans, available resources, and post-care expectations.
- Active and unrestricted New Mexico Registered Nurse (RN) license.
- Graduation from an accredited nursing program required.
- BLS certification required.
- At least 2 years of clinical nursing experience preferred; prior case management experience is a plus.
- Strong interpersonal and communication skills, with a patient-centered approach.
- Ability to work collaboratively in a team environment and independently manage a diverse caseload.
