Physical Therapist – Physical Therapy

Schedule: Days 5x8

Assignment Details:

• Contract Length: 13 weeks

Requirements:

• Graduate of an accredited Physical Therapy program
• Current Physical Therapist license in the state of practice
• Experience in skilled nursing facility or geriatric rehabilitation preferred

Roles & Responsibilities:

• Perform assessments of residents' mobility, balance, strength, gait, and functional limitations.
• Develop and implement individualized treatment plans to improve physical function and reduce pain.
• Provide therapy interventions, including exercises, gait training, and balance retraining.
• Monitor residents' progress and adjust care plans as necessary.
• Participate in interdisciplinary care team meetings and assist with discharge planning.
• Educate residents and caregivers on home exercise programs and safety strategies.
• Supervise and guide Physical Therapist Assistants and therapy aides.
• Document treatment sessions and progress notes in a timely manner.

Compliance / Notes:

• Ensure compliance with all state, federal, and facility documentation and practice standards.
• Support quality assurance and performance improvement initiatives as part of the rehabilitation team.
