School-Based Community Health WorkerThe Center for Human Development, (CHD) is seeking a part time School-Based Community Health Worker to join our Community School Based Clinic based in Franklin County.Your role as a School-Based Community Health WorkerThe School Based Tele Behavioral Health Program provides students with year-round long term or short-term counseling via tele and community resources. The Community Health Worker will work alongside a Clinician and school personnel to help students and families access services and resources. They will provide case management; ensuring students and families receive the help they need. They will document encounters; collect data and complete monthly reports. They will support with making referrals, obtaining releases of information, consents and other medical documentation.Requirements:High School Diploma or Equivalent requiredStrong computer skills- Electronic medical record (EMR) databases.Knowledge of area community agencies and resources.Skill in organizing resources and establishing priorities.Creative and analytical thinking.Strong interpersonal and communication skills.Essential job functions:The Community Health Worker will:Provide home, school and community-based care coordination to youth and their familiesAssist students and families to access on site tele behavioral counseling services as well as offsite community-based clinicians and non-clinical resources (as needed).Conduct comprehensive screening/assessment activities including mental health status, health screen and social needs factors, to support community-based referrals.Provide ongoing follow-up, motivational interviewing and goal setting with students/families.Pay range is $20-$25/hour based on experienceSchedule: Full time Monday-Friday, most hours within school day (8:00-3:00).At Center for Human Development (CHD) Care Finds a way:The Center for Human Development (CHD) provides a broad range of high quality, community-oriented human services dedicated to promoting, enhancing, and protecting the dignity and welfare of people in need.
