Cordial Health is seeking a compassionate and skilled Psychiatric Mental Health Nurse Practitioner (PMHNP) or Physician Assistant with psychiatric experience to provide on-site behavioral health care throughout Queens, New York .

This full-time position offers a stable weekday schedule, meaningful clinical work and the opportunity to build lasting relationships with an underserved adult and geriatric population.

Responsibilities include:

• Conduct comprehensive psychiatric evaluations and diagnostic assessments
  
• Provide ongoing psychiatric consultations and follow-up care
  
• Diagnose and manage psychiatric and behavioral health conditions
  
• Develop individualized, evidence-based treatment plans
  
• Prescribe, monitor and optimize psychotropic medications
  
• Evaluate medication effectiveness, side effects and behavioral stability
  
• Collaborate with physicians, nursing staff, therapists, social workers and families
  
• Participate in interdisciplinary treatment planning
  
• Educate residents, caregivers and staff regarding diagnoses and interventions
  
• Provide compassionate, trauma-informed and resident-centered care
  
• Maintain accurate, timely and compliant clinical documentation
  

• Board-certified PMHNP-BC or Physician Assistant with significant psychiatric experience
  
• Active and unrestricted New York professional license
  
• Active DEA registration required
  
• Strong knowledge of psychiatric assessment, diagnosis, psychopharmacology and treatment planning
  
• Ability to manage a psychiatric caseload independently
  
• Excellent communication, organization and relationship-building skills
  
• Geriatric, skilled nursing, rehabilitation, long-term care or post-acute experience preferred
  
• Familiarity with electronic medical record systems preferred
