Program Case Manager: LGBTQ+ Youth HIV Prevention

Upstate Medical University

SUNY Upstate Medical University in Syracuse is seeking a Program Case Manager to work within the Patient Services Team. You will address social determinants of health and offer resources to adolescents and young adults in need of sexual health and HIV prevention services. This role involves conducting assessments, developing individualized plans, and facilitating referrals to ensure culturally relevant support for patients.
