Monitor residents’ health, identify changes, and report to the healthcare team. Collaborate with the team to develop and implement care plans. Supervise and guide nursing assistants to ensure quality care delivery. Maintain accurate documentation and adhere to state and facility regulations. Provide emotional support and compassionate care to residents and their families. Qualifications Current Ohio LPN license in good standing. Experience in a long-term care or healthcare setting preferred. Strong communication, teamwork, and organizational skills. Compassionate, dependable, and attentive to resident needs.
