Accredited Pharmacy Technician Specialist

Summary: This role involves obtaining and updating medication histories for patients admitted or pending admission, ensuring accurate medication reconciliation within the electronic medical record system. The specialist prioritizes medication histories and gathers comprehensive medication information from various sources including patient interviews, healthcare providers, and pharmacies. Key Responsibilities:
Education: Completion of Pharmacy Technician Certification Board (PTCB) certification is required unless currently enrolled in a PharmD program. Registration with the state Board of Pharmacy is necessary. Experience: A minimum of 6 months to 1 year of experience as a pharmacy technician is required, with 2 years of retail pharmacy experience preferred. Skills and Abilities: Strong attention to detail and accuracy in technical functions, knowledge of common medication doses and forms, excellent interpersonal and communication skills, ability to prioritize in a high-volume environment, problem-solving skills, and adaptability to workload changes. Physical Requirements: Ability to work independently in stressful environments, read physician notes and medication labels, manual dexterity for operating pharmacy equipment, ability to stand and walk for extended periods (6-10 hours), and lift moderate loads (up to 10 lbs) with occasional stooping and kneeling.
- Navigate electronic medical records to prioritize and complete medication histories for admitted or pending patients.
- Collect detailed medication lists including drug name, dose, route, frequency, PRN indications, and last dose taken.
- Document medication and allergy information accurately and promptly in the electronic system.
- Refer unresolved medication issues to pharmacists and admitting providers.
- Maintain patient and staff confidentiality by securing records and releasing information only to authorized personnel.
- Report safety hazards promptly to supervisors or appropriate personnel.
- Conduct educational sessions for technicians, nurses, and other healthcare staff, including patient education.
- Precept and train pharmacy technicians, students, pharmacists, and nursing staff on medication history processes.
- Assist with data collection for performance improvement initiatives.
- Perform other duties as assigned.
