Job Types: Part-time, Contract, PRN Overview Boost Home Healthcare – Summerlin is seeking compassionate and skilled Therapists (PT, ST, OT & MSW) on a per diem basis to join our Home Health team for our 2026 opening! The ideal candidate will provide high‑quality nursing care to patients in their homes, ensuring their comfort and well‑being. Therapists will work collaboratively with a multidisciplinary team to manage individualized care plans tailored to each patient’s needs. The EMR utilized by our company is KanTime. Responsibilities Conduct comprehensive assessments of patients’ health status and needs in their home environment. Evaluate and manage individualized nursing care plans based on patient assessments. Administer medications and treatments as prescribed by physicians. Provide education and support to patients and their families regarding health management and disease prevention. Monitor patients for changes in condition and respond appropriately to emergencies. Maintain accurate documentation of patient progress and interventions in compliance with regulatory standards. Use KanTime EMR for documentation and care coordination. Qualifications Current valid license in the State of Nevada. Previous experience in home health care or a related field. Proficiency in critical care or urgent care is highly desirable. Strong interpersonal skills with the ability to communicate effectively with patients, families, and healthcare teams. #J-18808-Ljbffr Boost Home Healthcare
