POSITION DETAILS

• Locations Include: Albertville, Elk River, Otsego* (RN Only) & Surrounding Areas
• Hours (subject to change): Full-Time, Part-Time, and PRN opportunities available on days, evenings, nights, and weekends.
• Client Specifics: Kids ages 1-19 years old with trachs, vents, and g-tubes

Key Responsibilities:

• Provide direct nursing care to clients in home and school settings.
• Monitor patient conditions, perform assessments, and report changes to the care team.
• Administer medications, treatments, and prescribed therapies.
• Perform tracheostomy (trach) care, suctioning, and airway management.
• Monitor and manage ventilator-dependent patients and respiratory equipment.
• Administer feedings and medications through gastrostomy tubes (G-tubes) and provide site care.
• Maintain accurate and timely clinical documentation.
• Educate and support family members and caregivers regarding patient care needs.
• Respond appropriately to emergencies and follow established care protocols.
• Maintain a safe, clean, and therapeutic care environment.

TRAINING & SUPPORT

We’ll help you breeze through the learning curve and into a role that truly makes a difference. With 24/7 support from our dedicated team, you’ll never feel alone. Pediatric experience is a plus, but if you’ve got heart and a willingness to learn, you’ll fit right in with our warm and welcoming crew!

Night & Weekend Bonus Structure - You choose the shift. We add the boost.

• Weekday Nights (Mon–Fri): +$1/hour
• Weekend Nights (Sat–Sun): +$2/hour
• Weekend Days (Sat–Sun): +$1/hour

SALARY RANGE

At PHS, we are committed to providing specialized care for medically complex children. Every role, from direct patient care to customer support, plays a vital part in improving the lives of kids and families. The impact you make is invaluable. We are dedicated to offering fair and transparent compensation to our team members. The base range for this role is $25.00 to $31.00 hourly for LPN and $35.00 to $41.00 hourly for RN , which is based on license, relevant experience, and qualifications.
