Community Staff Nurse - Shrewsbury South (Fixed-Term) NHS AfC: Band 5 Main area: Community Nursing Grade NHS AfC: Band 5 Contract: Fixed term – 12 months (or a secondment opportunity if internal) Hours: Full time, Flexible working, 37.5 hours per week Job ref: View phone number on click.appcast.io-OPS-A Job overview An exciting opportunity for a highly motivated individual to join our Community Nursing Team covering the Shrewsbury South locality in the provision of planned care to patients in the community. This is a full‑time, 37.5 hour per week, fixed‑term position lasting 12 months. Main duties of the job Provide nursing care to patients within their own homes or chosen place of residence, offering an alternative to hospital admission, supporting timely discharge and enabling people to realise their potential. Support clinical care, CPD and training, and cover a 7‑day, 8:00 am to 6:00 pm service. Person specification Registered with the RGN and the NMC Demonstrates a level of English language proficiency suitable for writing service‑user case notes Successful completion of mandatory training and up‑to‑date with evidence‑based practice Full driving licence and access to a vehicle for work purposes Experience Experience working in a multi‑disciplinary / multi‑agency health/social care environment Experience and knowledge of basic administrative systems including Microsoft Office Successful partnership working arrangements with other services Previous community nursing experience Knowledge and Skills IT skills or willingness to undertake RIO training Uses evidence based practice Works within own defined competencies Awareness of changing healthcare factors influencing change; demonstrates tact and diplomacy Well refined communication (verbal and written), interpersonal skills – ability to gain rapport at all levels Engaging and empowering approach that seeks to work with others and deliver outcomes in a successful and well‑informed manner Employer certification / accreditation You must have appropriate UK professional registration and meet the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020, including disclosure to the Disclosure and Barring Service.
