Role Description This is a full-time, on-site Licensed Practical Nurse (LPN) role based in Fort Walton Beach, FL. The LPN will provide direct nursing care, including medication administration, treatments, and monitoring of vital signs, in accordance with established care plans and clinical guidelines. Daily responsibilities include observing and documenting client conditions, reporting changes to supervising registered nurses or physicians, and supporting the development and implementation of individualized service plans. The LPN will collaborate with interdisciplinary teams, assist with health education for individuals and caregivers, and help maintain a safe, clean, and supportive environment. The role also involves ensuring compliance with regulatory and company policies, participating in team meetings, and supporting quality improvement efforts. Qualifications Active and unrestricted Licensed Practical Nurse (LPN) license in the state of Florida. Demonstrated skills in direct patient care, including medication administration, basic wound care, and monitoring vital signs. Ability to accurately document care, follow care plans, and communicate changes in condition to the clinical team. Strong interpersonal and communication skills for working collaboratively with individuals, families, and multidisciplinary staff. Knowledge of infection control, safety procedures, and regulatory requirements applicable to community-based care settings. Capacity to work on-site, manage multiple priorities, and respond effectively in urgent or emergent situations. High school diploma or equivalent; completion of an accredited practical nursing program required. CPR certification and ability to meet any additional health, background, and training requirements established by the organization or state.
