Home Health Aide Patient Escort Brooklyn Center is actively seeking Home Health Aides to serve as Patient Escorts for our Skilled Nursing Facility in Brooklyn, NY.

DUTIES:

Escort residents to and from appointments Observing and monitoring Residents. Reporting any health issues to the supervising nurse. Maintain confidentiality of all Resident & Facility data.

REQUIREMENTS:

Valid New York State HHA Certification. Occasional heavy lifting may be required. Ability to follow the Plan of Care precisely!
