Per Diem LPN

The Manor Health & Rehabilitation Center has an employment opportunity available for a Per Diem LPN. This role provides skilled nursing by performing assigned duties and services required to deliver quality resident care. The position follows established standards, Joint Commission requirements, and CentraState policies and procedures.

Responsibilities

The responsibilities include:

• Providing considerate respectful care focused upon the Residents' individual needs.
• Demonstrating the ability to obtain information and interpret information in terms of the needs of the age of the Residents cared for.
• Demonstrating knowledge of growth, development and/or adult health throughout the life span as appropriate to the job performed.
• Correctly performing all scheduled and assigned tasks while continually observing Resident needs.
• Delegating and monitoring components of care to professional and nonprofessional caregivers as appropriate.
• Implementing basic nursing care and assisting Residents with activities of daily living.
• Accurately collecting Resident data utilizing learned technical skills under the direction of the RN.
• Making decisions that demonstrate knowledge of nursing practice and utilizing sound judgment in clinical and Resident care situations.
• Participating in preparation and intervention of diagnostic or therapeutic Resident care.
• Demonstrating accountability for administration, observation, charting and reporting effects of drugs, intravenous, other therapeutic and diagnostic measures in a safe and accurate manner.
• Administering medications following CSHS policies and manufacturers' instructions.
• Recognizing adverse reactions to medications and taking immediate and appropriate actions.
• Following CSHS policy regarding restraint application, documentation and observation.
• Consistently including Resident and family in the development of an individualized plan of care.
• Performing pain (5th vital sign) screening as indicated, notifying the RN immediately of patient's report of pain or discomfort, and providing appropriate non-pharmacologic interventions as directed by RN.
• Documenting pain assessment, intervention and follow-up.
• Documenting reassessment of pain as per hospital policy.
• Providing Resident/family education regarding pain.
• Notifying physician for unrelieved pain as indicated.

Qualifications

The qualifications include:

• Graduate of an approved program for licensed practical nurses and licensed to practice in NJ.
• Previous LPN experience preferred.
• Valid New Jersey LPN license required.
• CPR certification required.
• Satisfactory completion of the pharmacology test is required.
• Strong professional, organizational, and interpersonal skills to effectively relate with all members of the healthcare team.

Compensation Range: $27.00 - $43.20 per hour

The compensation above reflects the established range from CentraState Healthcare System (CSHS) for this position at the time the job was posted. CSHS considers many factors to determine compensation, including education, experience, skills, licenses, certification, and training. As such, team member compensation may fall outside this range.

Additionally, the compensation range reflects base salary and does not include extra shift rates or incentives tied to quality, productivity, etc., as applicable.
