Responsibilities Provide outpatient rehabilitation services in an in‑home setting for older adults. Evaluate and treat patients in the most functional environment, often at the patient’s residence or independent living facility. Identify therapeutic needs, develop and deliver individualized physical therapy programs to facilitate rehabilitation. Make referrals to other disciplines and agencies as needed. Implement individualized therapy programs to restore, reinforce, and enhance performance. Instruct, educate, and train patients and caregivers in skills essential for independence and productivity. Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel, following state licensure and professional standards. Ensure adherence to department and patient treatment schedules, timely clinical and billing documentation, and participation in patient care and staffing conferences. Audit safety, eliminate hazards, order supplies, maintain equipment, and report repairs as needed. Maintain a patient‑first approach, ensuring the highest quality, caring, compassionate service. Screen, examine, and evaluate patients: history, systems review, apply appropriate tests, interpret findings, establish diagnosis, identify impairments, predict level of improvement, set goals, and create plan of care. Apply therapeutic techniques, including: therapeutic exercise, functional training, manual therapy, airway clearance, integumentary repair, electro‑therapeutic modalities, health and wellness, education on chronic disease management, falls assessment, case management, and adaptive device design and training. Incorporate health and wellness programming into treatment plans. Perform home and community mobility assessments and recommend safe mobility strategies. Other related duties as required. Qualifications Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility. Master’s degree in Physical Therapy, or a related field with Bachelor’s in Physical Therapy, or Bachelor’s in Physical Therapy with five years of progressively responsible PT experience. Medical degree earned outside the U.S. must be evaluated as equivalent to a U.S. Physical Therapy degree.
