Roles & Responsibilities
- Evaluate residents and create personalized treatment plans to improve their functional independence
- Implement therapeutic interventions under physician orders, ensuring safety and efficacy
- Collaborate with the interdisciplinary team to coordinate care and discharge planning
- Document progress in the electronic medical record with attention to Medicare and other regulations
- Educate residents and caregivers on home exercise programs and assistive devices
- Supervise and mentor Physical Therapist Assistants and new staff
Qualifications
- Active Physical Therapist license in Massachusetts required
- Minimum 3 years of experience in a skilled nursing or long-term care setting required
- BLS certification preferred (but we can help you get it)
- Strong communication and teamwork skills to thrive in a collaborative environment
Education
- Bachelor's degree in Physical Therapy required; Master's or Doctorate preferred
Knowledge & Skills
- Expertise in patient evaluation, treatment planning, and rehabilitation techniques
- Proficiency in Microsoft Office and electronic medical records
- Excellent time management and documentation abilities
- Compassionate, resident-centered approach to care
