Medical Director For The Home-Based Primary Care Program

The Medical Director for the Home-Based Primary Care Program (HBPC) is a physician with skills and experience in providing primary care to veterans in their home and experience interest in geriatrics and palliative/hospice care, who is responsible for the overall medical care delivered by the HBPC team. This position is approximately 40% administrative and 60% clinical. The clinical work will be divided between in-home care for veterans enrolled in the HBPC program and support to the long-term care unit coverage. This position will have an assigned panel of patients/veterans from within the Tomah HBPC program. In addition to providing primary health care, the physician serves as a consultant to nursing, and medical staff, and interdisciplinary team members in providing quality care. The physician serves on medical center, VISN and/or national committees as appropriate, and assists in the development, evaluation, and communication of policies and standards to all healthcare disciplines.

Pay: Competitive salary, annual performance bonus, regular salary increases
