Serving as a bridge between outpatient care and crisis services, this role delivers timely interventions, assesses risk and level of care needs, and facilitates connection or reconnection to ongoing behavioral health treatment.The position also supports transitions of care through hospital and crisis discharge follow-up and collaborates closely with internal clinical teams, Crisis Services, hospitals, and community providers. Services may be provided in person or via telehealth. This role requires the ability to manage a dynamic caseload, respond to unplanned clinical needs, make timely decisions regarding risk and disposition, and provide focused, short-term interventions rather than long-term psychiatric management.Duties include:Rapid Psychiatric Assessment and Triage• Provides timely psychiatric assessment for individuals experiencing acute or emerging behavioral health concerns.• Evaluates psychiatric symptoms, current treatment, medication needs, substance use, psychosocial factors, medical considerations, and other factors contributing to the current presentation.• Performs suicide, self-harm, violence, and other relevant safety and risk assessments.• Determines the appropriate level of care based on clinical presentation and risk.• Identifies individuals who can safely receive outpatient intervention and those requiring escalation to Crisis Services, emergency department evaluation, 23-hour observation, inpatient hospitalization, detoxification, or another higher level of care.• Recognizes medical conditions or substance-related presentations requiring medical evaluation rather than outpatient psychiatric intervention.Short-Term Treatment and Stabilization• Provides focused psychiatric intervention intended to stabilize acute symptoms and reduce unnecessary utilization of higher levels of care when clinically appropriate.• Develops short-term treatment and safety plans based on the individual’s immediate clinical needs.• Initiates or modifies psychiatric medications when clinically appropriate.• Provides urgent medication refills and bridge prescriptions when interruption of treatment could contribute to clinical deterioration.• Provides limited bridge care when necessary while an individual is being connected or reconnected with an ongoing psychiatric provider.• Engages individuals and, when appropriate, families, guardians, or other supports in the immediate treatment and stabilization plan.Crisis Diversion and Disposition• Collaborates closely with Crisis Services to evaluate individuals whose needs may be appropriately managed outside of a 23-hour observation or inpatient setting.• Provides clinical consultation regarding psychiatric presentation, medication concerns, risk, and appropriate disposition when needed.• Facilitates escalation to a higher level of care when outpatient management is no longer clinically appropriate or safe.• Supports effective transitions between crisis, hospital, outpatient, and community-based behavioral health services.• Participates in follow-up processes for individuals recently evaluated through Crisis Services when clinically indicated.Hospital and Crisis Discharge Follow-Up• Provides timely psychiatric follow-up following discharge from psychiatric hospitalization, emergency departments, crisis facilities, or other acute behavioral health settings.• Reviews available discharge information and reconciles psychiatric medications.• Assesses current psychiatric stability, medication adherence and tolerability, safety concerns, and ongoing treatment needs.• Addresses immediate post-discharge psychiatric needs and facilitates connection or return to the individual’s ongoing treatment provider.• Identifies individuals whose post-discharge presentation requires more intensive intervention or a higher level of care.Connection to Ongoing Care• Facilitates timely handoff to established Terros Health providers, other internal programs, existing community providers, or new ongoing treatment resources.• Works to reconnect individuals who have become disengaged from established psychiatric treatment.• Coordinates with clinical teams to minimize gaps in psychiatric care.• Clearly communicates assessment findings, medication changes, risk considerations, and follow-up recommendations to the receiving treatment team.• Maintains a short-term, transition-focused model of care rather than developing an ongoing longitudinal caseload except when clinically or operationally indicated.Interdisciplinary Collaboration• Works collaboratively with Crisis Services, psychiatric providers, nursing, case management, therapists, clinical leadership, hospitals, emergency departments, and community partners.• Provides psychiatric consultation to clinical teams regarding urgent presentations and appropriate level of care when appropriate.• Participates in development and ongoing improvement of workflows related to crisis referrals, urgent psychiatric access, hospital discharge, follow-up, and transition to ongoing services.• Coordinates with other members of the treatment team to address barriers to successful follow-up, including transportation and access to services when applicable.Clinical Practice and Documentation• Completes timely, accurate, and clinically appropriate documentation of psychiatric assessments, risk assessments, treatment decisions, medication interventions, disposition, and follow-up recommendations.• Prescribes medications in accordance with applicable federal and state regulations, professional standards, and Terros Health policies and procedures.• Practices within established standards of care and the scope of practice for a Psychiatric Mental Health Nurse Practitioner.• Maintains appropriate communication with referring and receiving providers to support continuity and safety of care.• Participates in quality improvement activities related to urgent psychiatric access, transitions of care, crisis diversion, and clinical
