ABOUT THIS COMMUNITY HallMar Village is a joint ministry of Mercy Medical Center and Presbyterian Homes & Services. This senior living community offers independent living, assisted living, and memory care apartment homes as well as long term care suites. With the Village Center at its heart, the community offers inviting and comfortable living spaces along with outdoor elements including patios, courtyards, gardening spaces, and walking trails. This community offers the following care options: Independent Living, Assisted Living, Memory Care, Care Center/Long Term Care Address: 8900 C Ave NE, Cedar Rapids, IA 52302 Responsibilities As an Occupational Therapist you will be responsible to: Conduct a comprehensive assessment & evaluation using subjective and objective measures. Develop treatment plans and programs in accordance with physician orders. Provide quality treatments, actively progressing goals to achieve highest quality of life. Conduct screens and provide education to residents/clients, families, caregivers, and staff. Coordinate seamless discharges utilizing the full continuum of therapy and fitness services. Provide accurate and timely documentation & billing in electronic medical record. Demonstrate understanding and knowledge of payer sources and regulatory requirements. Complete and document supervisory visits per regulations. Communicate effectively with residents/clients, families, caregivers, physicians, referral sources, and interdisciplinary team. Qualifications Graduate from an accredited Occupational Therapy program. Current licensure in the state(s) of practice as required. Demonstrated compatibility with PHS's mission and operating philosophies. Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.
