Supervisor - Case Management

Job Description Directly supervise registered nurse case managers and social workers, ensuring high‑quality patient care coordination, utilization review, and staff development in alignment with the organization’s mission. Education Required: Bachelor’s degree in nursing or related healthcare discipline OR Master’s degree in Social Work (MSW). Preferred: Master’s degree in Nursing (MSN) OR Licensed Certified Social Worker (LCSW). Work Experience Required: 3 years of utilization and/or case management experience with a registered nurse license OR Master of Social Work (MSW). Preferred: 3 years of supervisory or leadership experience in a hospital environment. Certifications Required: Current registered nurse license in the state of practice OR Current Master of Social Work (MSW) license. Knowledge, Skills, and Abilities Leadership skills fostering teamwork, professionalism, customer satisfaction, high performance, and mutual respect. Effective verbal and written communication for diverse audiences throughout the patient care process. Computer skills for data entry, retrieval, and multitasking. Knowledge of the welfare system. Financial acumen to analyze trends, recommend actions, and manage budgets. Evaluation of patient care data to ensure compliance with clinical guidelines and standards. Conflict resolution and problem‑solving skills. Job Duties Guide care management staff on daily operational performance matters. Supervise case managers and social workers responsible for patient care coordination, utilization review, intake and discharge planning. Utilize financial and performance data to align staff with organizational goals. Serve as contact and resolution provider for customer service matters. Educate staff on patient experience to ensure outstanding customer service. Facilitate and contribute to leadership meetings within and beyond the department. Oversee onboarding and annual staff performance evaluations. Adapt communication style and behavior to specific patient populations, ensuring privacy and clarity. Perform other related duties as required. Physical and Environmental Demands Medium work with occasional lifting of 20 to 50 pounds, frequent lifting of 10 to 25 pounds, and/or lighter loads. Exposure to infectious agents and hazardous materials may occur. The role is performed in a healthcare setting, potentially including isolation rooms.
