Independent Contract- Catastrophic Field Nurse Case Manager

What You’ll Do as part of a team:

Assess patient needs and develop personalized care plans after visiting them in the hospital, home or rehabilitation settings.
Coordinate with healthcare providers, families, and community resources to ensure seamless care.
Monitor patient progress and adjust plans as necessary to meet evolving needs.
Advocate for patients, ensuring they receive the best possible care.
Provide education and support to patients and their families regarding treatment plans and health conditions.

What We’re Looking For:

Current RN License (in the state you’ll be working).
Current, unrestricted driver's license and auto insurance
Ability to travel within 100+ miles of your home for patient appointments, hospital and rehabilitation visits
Minimum of 2 years of trauma or ICU nursing experience , with some experience in case management.
Ability to work independently and manage your schedule effectively.
Comfort with computer skills and healthcare software data entry
A highly organized individual with a focus on timeliness and attention to detail
